After your hospital stay, your physician may order home health. Enjoy all the benefits of a professional health care team in the comfort of your own home.
Why Home Health?

If you are recovering from an illness, accident, or need help learning to better manage chronic illness or disability, our professionals will teach you and your caregiver what you need to know about getting well.
Skilled Nursing Care
Nursing care is directed by your physician’s orders. Home health care is intermittent, not continuous, not 24 hour care. A nursing assessment determines how often care will be provided. Provided By:

• Home Health Aides
• Licensed Vocational Nurses
Rehabilitation Care
Highly skilled therapists develop a specialized rehabilitation plan for each patient. Provided by:
• Licensed Physical Therapists
• Licensed Speech Therapists
• Physical Therapy Assistants
• Licensed Occupational Therapists
Supportive Services
Maintain your independence with support, counseling as well as connection to community resources provided by our Medical Social Workers.
Skilled Nursing Services
May Include the following:
• Wound care
• Chronic disease management
• Infusion therapy
• Catheter care |
• Pain and symptom management
• Medication instruction, monitoring
• Bowel management
• Skin integrity monitoring |
Admission Criteria to Receive Home Health Care
• A physician order is required and your physician must follow your care while you are on service.
• You must have a skilled need, something that only a health care professional can provide or teach
• You are homebound, cannot leave home but for short periods of time, with difficulty
• You are able to care for yourself or you have a caregiver
• A place of residence to receive services

Home health is part of your health care benefit
whether you have Medi-care, most Private Insurance and most HMO’s and PPO’s.
When home care is ordered the following services can be coordinated as directed by physician:
• Infusion therapy
• Oxygen
• Medical equipment
